Forehead lines: what causes them and how they’re treated
What causes them
The frontalis is a broad sheet of muscle across the forehead, and it has exactly one job: lifting the eyebrows. Every time you raise them — in surprise, in conversation, or unconsciously to open your eyes wider — the skin above bunches into horizontal folds.
Because it is the only brow elevator, it also works overtime to compensate for heaviness in the upper lid, which is why forehead lines often deepen noticeably in the late thirties and forties.

What we would recommend
A wrinkle relaxer, dosed conservatively. Typical ranges for the forehead are 8 to 20 units of BOTOX®, Xeomin® or Jeuveau®, or 20 to 30 units of Dysport® — but the correct number is determined at your face, after your injector has evaluated how strong the muscle is and how much brow lift you rely on.
One point matters more here than anywhere else on the face: the forehead is almost never treated in isolation. The frontalis lifts the brow and the glabellar muscles between the brows pull it down. Relax the lifter without addressing the depressors and the brow can sit heavier than it did before. Treating the two areas together is the norm, not an upsell.
What it won't fix
A relaxer will not lift a heavy brow or a hooded upper lid. If anything, over-treating the forehead makes both look worse, because you have removed the muscle that was compensating. That is a surgical problem, and we will tell you so rather than inject around it.
It will not fully erase a deep furrow that is clearly visible with your face at rest — those have a static component in the skin itself, and the honest expectation is significant softening rather than a flat forehead, improving over consecutive treatments.
And it will not last longer than three to four months. Nothing injectable does.

How many units are needed for forehead lines?

Why do I need my glabella treated as well as my forehead?
The forehead muscle lifts the brow and the glabellar muscles between the brows pull it down. Relaxing only the lifter can leave the brow sitting heavier than before. Treating both areas together keeps the balance.
Can treating my forehead give me a heavy or droopy brow?
It can if it is over-dosed or dosed without accounting for how much you rely on the frontalis to hold your brow up. This is why the assessment matters more than the price per unit, and why a conservative first treatment that can be topped up is safer than a large one that cannot be undone.
How soon will I see a difference?
Results often take 2 to 10 days, and sometimes up to 14 days, to take effect. Judge the outcome at two weeks.
